What You’ll Do
- Provide a wide scope of activities for Developmental Disabilities Support Coordination services for a two-county area.
- Work closely with program staff to provide initial and ongoing training.
- Audits the accuracy and timeliness of ISPs, UR requests, case notes, budget, and other documents.
- Attends interdisciplinary team meetings to discuss client cases in relation to eligibility, support plans, progress, and possible changes in the service program.
- Conduct pre-survey reviews to identify and correct deficiencies before SMH/CMS/MMAC reviews.
- Affords assistance to Program Managers in gathering data for reviews.
- Develops and maintains flowcharts and spreadsheets, tracking data and reporting findings to the team.
- As needed, manages a selected caseload of clients with intellectual disabilities/developmental disabilities in placement or natural homes.
- Performs other related duties as assigned.

Requirements
- Required Minimum Qualifications: Bachelor’s degree in social work (BSW) or equivalent human services degree; OR Registered Nurse (licensed)
- Two (2) years of experience in case management in a human service field.
- This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
- Preferred: Previous targeted case management experience.
