Job Responsibilities
- Support people to lead fulfilling lives with a sense of belonging, purpose, and meaningful relationships.
- Treat all people supported with dignity and respect.
- Provide guidance and support to ensure successful participation in people’s activities of their choosing and achievement of their goals based on their Person-Centered Plan.
- Complete accurate and quality daily documentation and reporting related to supported individuals’ activities and goals progress.
- Provide personal care assistance as needed.
- Participate in the development of Person-Centered Plans by communicating preferences, strengths, needs and barriers of persons supported to their teams.
- Provide, monitor, and maintain a safe work environment.
- Provide behavioral support and medication administration, if appropriate, per the supported person’s plan.
- Drive an agency vehicle or your personal vehicle, as appropriate, to transport people supported to activities in the community or for agency related business as needed.
- Ensure monthly vehicle logs, mileage forms (if applicable) are completed and turned in on schedule.
- Complete and maintain all agency-required training per established guidelines.
- Perform other duties as assigned by supervisor.
Qualifications
- Must be independent and self-directed.
- Must be at least 20 years of age.
- Valid Maryland driver’s license, clean driving record and three (3) or more years of driving experience.
- High School Diploma/GED preferred.
- Two (2) or more years of experience working with adults with developmental disabilities preferred.
- CMT certification desirable.
