Responsibilities

  • Perform the delivery of services with this organization and our consumers
  • Provide transportation of consumers to and from day programming, community activities, and appointments
  • Perform all daily activities within day program setting
  • Assist consumers with activities of daily living
  • Assist with ISP development and outcomes
  • Accurate completion of required paperwork to include but not limited to timecards, progress notes, medication logs, vehicle logs, and expense forms
  • Be available for shifts twenty-four (24) hours a day, seven (7) days per week as scheduled
  • Assist with on call activities and emergency situations as requested by the Program Coordinator
  • Responsible for the safety and well-being of all the individuals participating in day services
  • Work with Program Coordinator to ensure that all the needs of the individuals are met according to their ISP
  • Assist the Program Coordinator with special projects as related to the Agency
  • Observes all ATP operating regulations

Requirements

  • High School Diploma or GED
  • Six (6) months to one year of experience in Human Services
  • Excellent Communication skills
  • Excellent writing and documentation skills
  • Team Player

Preferred Qualifications

  • Experience working with individuals with intellectual and developmental disabilities

Physical Requirements

  • Ability to sit, stand, walk, bend, stoop, reach, lift, and climb stairs as needed
  • Ability to drive a vehicle and travel frequently within assigned service areas
  • Ability to operate standard office equipment and electronic documentation systems
  • Ability to perform essential job functions with or without reasonable accommodation
