About the role

Community Bridges is looking for a 1 part-time Direct Support Professional in Penacook, NH.

Responsibilities

  • Aid individuals in developing a sense of self-esteem and confidence through planned activities and community involvement.
  • Provide individualized support including personal care and skill acquisition.
  • Help individuals establish, maintain, and present a positive image.
  • Support the development and individualization of yearly Person-Centered Planning by providing necessary information and assisting with goal/objective implementation strategies and method monitoring.
  • Complete documentation requirements as outlined in the Individual Service Agreement, including scheduled data collection and monthly progress notes.
  • Ensure compliance with all company and state required trainings, including Relias and in-person trainings, and annual ISA sign-offs.
  • Report any safety, health, or human rights concerns immediately to the supervisor.
  • Comply with applicable state regulations.
  • Perform other duties as assigned.

Requirements

  • High School Diploma or GED.
  • Experience in the human service or related field with persons experiencing disabilities is desirable.
  • Reliable transportation, valid driver’s license, and proof of automobile insurance.

Compensation and Benefits

Specific Duties and Responsibilities

  • Ability to represent Community Choices and its services to the community with a positive and professional image.
  • Ability to demonstrate a positive attitude under stressful situations with individuals, co-workers, and people external to the organization.

Environment and Physical Demands

  • Work environment: Typically in the community. Required to attend in-person trainings and meetings at the office.
  • Physical demands: Must be physically able to lift up to 50 lbs., provide transfers if necessary, walk and stand frequently, push wheelchairs, operate wheelchair lifts/Hoyer and barrier-free lifts if required for clients, and perform emergency behavioral interventions as per behavioral intervention techniques and individual behavioral plans.

Travel

  • Must have access to a personal vehicle during all scheduled work hours.
  • Frequently travels with supported individuals utilizing personal vehicle.
  • Must possess and maintain a valid NH license and safe driver's record in accordance with Community Choices standards with adequate liability insurance.
