DAY HABILITATION PROVIDER

Duties & Responsibilities

• Ensures consumer safety by providing consistent supervision and observation.
• Promotes a homelike atmosphere, in the Residence and a nurturing learning environment in the Day Habilitation Program.
• Implements Individual Habilitation Plans and provides training of adult living skills in a dignified and respectful manner.
• Performs tasks on-site and in the community that involves training developmentally disabled individuals in various life skill areas.

Requirements

• Successfully complete AMAP Certification and administer medications as prescribed by a doctor (s) to the individuals as soon as scheduled, within 90 days of hire.
• Successfully complete Tube feeding training, Diabetic Training, Colostomy training, and any specialized certification, if required, within 90 days of hire.

Physical Requirements

• Regularly required to communicate clearly and effectively with staff and consumers.
• Frequently required to stand for long periods of time; use mobility; restrain consumers when necessary and reach for objects.
• Occasionally required to sit and stoop, kneel and crouch.
• Must occasionally lift and/or move up to 25 pounds.
• Light duty is not permissible.
• Must be able to go up and down approximately 20 stairs to the 2nd floor bedrooms, 3.6 times a day at a minimum.
