About the role
The Day Habilitation Specialist is responsible for providing individualized support and services to children and adults with developmental disabilities in a community-based setting. This position requires strong communication skills, patience, and the ability to work collaboratively with families, caregivers, and other professionals.
Responsibilities
- Develop and implement individualized service plans based on assessments and goals
- Provide direct support to individuals in activities of daily living, socialization, and educational activities
- Collaborate with families and caregivers to ensure consistency and continuity of care
- Document progress and communicate with team members and families regularly
- Participate in staff meetings and training sessions
Requirements
- Bachelor's degree in a related field (e.g., Special Education, Psychology, Social Work)
- Experience working with individuals with developmental disabilities
- Valid driver’s license and access to reliable transportation
- Ability to pass a background check
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Knowledge of community resources and supports for individuals with developmental disabilities
- Excellent organizational and time management skills
- Proficiency in Microsoft Office Suite
- Ability to work flexible hours, including evenings and weekends
