Day Center CNA

Responsibilities

The Day Center CNA (Certified Nurse Assistant) works at our Adult Day Health Center, assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Responsibilities include:

• Following the care plan and assisting participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
• Documenting care provided in accordance with established policies, guidelines, and applicable regulations.
• Carrying out, reporting, and documenting care given in an effective, timely manner as observed by the RN and through periodic record reviews.
• Assisting participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, filling oxygen, and providing other aides as assigned or needed.
• Monitoring participants' health status, including vital signs, weights, skin condition, appetite, and changes in functional ability, and reporting pertinent information to the clinical supervisor in a timely manner.
• Collaborating with multidisciplinary teams in care planning for participants.
• Monitoring appetite and looking for changes in functional ability and skin condition.
• Monitoring/assisting participants in a courteous manner with meals and helping with feeding, as necessary.
• Reporting any variance, accident, or unusual occurrence to the appropriate personnel.
• Assisting with loading and unloading of participants from vehicles.
• Assisting the transportation department with participants’ take-home supplies.
• Serving as a bus attendant on company vehicles while InnovAge drivers transport participants to/from the center or medical appointments, supporting participants while in transit and accompanying them in and out of facilities.
• Creating and participating in group(s) and one-on-one activities in accordance with the daily activity calendar when needed.
• Attending in-service and continuing education programs as scheduled and necessary.
• Performing other duties as assigned.

Requirements

• Active and valid state-issued Certified Nursing Assistant (CNA) license.
• Current CPR certification or the ability to obtain CPR certification from acceptable vendors (American Heart Association and/or American Red Cross).
• High School Diploma or GED equivalent.

Preferred Qualifications

• Experience in a care setting for frail and elderly patients participating in a group environment.
• Bi-lingual.

Pay

Posted Pay Range: $19.76 - $22.74

The pay may vary depending on job-related factors such as work location, experience, knowledge, skills, education, certifications, training, and internal equity.
