About the role
Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute, and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community. This position qualifies for a $2500 sign-on bonus with a 2-year commitment, paid out in installments over the commitment period. The role is primarily based at the LIFE St Mary facility in Feasterville-Trevose, PA, with occasional home care work.
Responsibilities
- Provide personal care assistance maintaining the independence of participants to conduct activities of daily living, including toileting, showers, eating, and grooming.
- Assist participants in the maintenance of physical activity, exercise, and walking.
- Provide behavioral supervision or support for participants, such as reality orientation, reminders to follow nursing and medication instructions, diet restrictions, and emotional reassurance.
- Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.
Requirements
- Must be an active certified nurse assistant in the state where the PACE Organization is located.
- Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred.
- Valid driver’s license.
- Superior written/verbal communication skills.
- Must be customer-focused, exercise independent judgment, and demonstrate flexibility to adapt to ongoing change.
- Proficiency with Microsoft Office (Word, Excel, PowerPoint) and basic knowledge/experience with email and calendaring systems.
- Position may require 10% local travel and use of a personal vehicle.
- Ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending, and lifting. Must be able to push, pull, move, and/or lift a minimum of 20 pounds.
