ESSENTIAL JOB FUNCTIONS
- Assist with activities of daily living (ADLs), including mobility, toileting, grooming, and personal hygiene, to support participants in maintaining dignity, comfort, and personal well-being.
- Offer attentive supervision and support to ensure participant safety while promoting independence and confidence in daily routines.
- Aid participants during meal service and provide hydration reminders to help maintain proper nutrition and overall health.
- Encourage and assist with group and individual activities that foster social interaction, meaningful engagement, and a positive program experience for participants.
- Build respectful and supportive relationships with participants to promote inclusion, participation, and emotional well-being.
- Observe participants for changes in physical condition, behavior, or emotional well-being and promptly communicate relevant information to Day Break Supervisor to ensure timely support and intervention.
- Maintain clear and professional communication with team members, participants, and families to support coordinated and participant-centered care.
- Completes comprehensive orientation and ongoing training as directed by the organization. Proficient and willing to perform CPR and Basic First Aid. Annual Tuberculosis (TB) assessment is mandatory. Maintains strict confidentiality regarding sensitive information.
- Maintain a clean, organized, and safe environment that supports participant comfort and reduces potential safety risks.
- Accurately document care provided and observations in accordance with program policies to ensure compliance, continuity of care, and effective team communication.
Physical Requirements
Demonstrates a full range of physical motion as required by the tasks. Regularly lifts, transfers, and positions objects weighing up to 50 pounds without assistance. Utilizes mechanical lifts to lift objects weighing more than 100 pounds.
