About the role
Provide CTI services for assigned youth, developing relationships with assigned youth/families, developing relationships with hospitals and service providers, following the fidelity of the CTI model, completing assessments, progress notes, and reports, as required.
Responsibilities
- Meet with youth and family referred from the hospital for CTI services (at home, in the hospital, or in the community)
- Complete intake assessment to determine needs and goals
- Provide intensive case management for the assigned youth/family for a 9-month period
- Assist the youth/family in determining additional services needed to achieve and maintain stability
- Develop a schedule of visits with the youth/family based on CTI requirements and the youth/family’s needs
- Ensure that all required documentation is completed and maintained per agency/department standards
- Meet with CTTP Director as scheduled to discuss CTI implementation, timelines, and progress of each youth
- Participate in the on-call rotation for crisis intervention support
Requirements
- A Bachelor’s degree in a Human Service field is required.
- One year of experience working with people with developmental disabilities or a mental health diagnosis or case management experience working with youth and families is preferred.
- The position requires good verbal and written communication skills.
- Candidates must be able to demonstrate teamwork and professionalism.
- Basic word processing and e-mail skills are required.
Qualifications
- Candidates must have a valid NYS Driver's License in accordance with the Agency Vehicle and Driving policy and have transportation necessary to meet program needs.
- The role requires on-call availability for bona fide emergencies and after-hours program service needs.
- Schedule flexibility is required, including occasional early morning, evening, or weekend meetings/functions.
- Candidates must be willing to meet with youth/family in hospital, home, and community settings.
