About the Role
Provide crisis and other outpatient services to children, youth, families, and adults to improve the quality of life for the individuals and families served.
A $2,500 retention bonus is paid upon completion of six months of employment, provided the employee remains in good standing.
Supervision
This position is supervised by a Program Supervisor or other identified member of management.
Responsibilities
- Promotes a warm and engaging organizational culture aligned with NorthCare values, trauma-informed care, and health and wellness.
- Provides crisis management services, including face-to-face or telephone interviews, lethality assessments, evaluation of support systems, and arranging appropriate interventions such as hospitalization.
- Completes appropriate assessment tools when potential for harm exists and facilitates referrals to the appropriate level of care in consultation with management.
- Consults with clinical leadership to assist in risk management and address clinical disruptions.
- Coordinates intervention, outreach, and referrals to ensure continuity of care, documenting all crisis services in Avatar within 24 hours.
- Follows up with consumers within 24 hours after a crisis to ensure safety; continues follow-up attempts daily for at least 5 business days if no contact is made.
- Coordinates and completes follow-up on all reported crisis services following best practices and within payor timeframes.
- Provides integrated services addressing behavioral healthcare, physical healthcare, and social needs that are recovery-focused, strengths-based, culturally competent, client-centered, trauma-informed, and co-occurring capable.
- Provides brief evidence-based counseling to individuals or families and group therapy or other supportive services (e.g., case management, care coordination, wellness) as needed.
- Consults with other program staff within and outside the agency to facilitate referrals.
- Performs screenings, intake interviews, treatment plan reviews, and initial assessments, gathering information about symptoms, behavior, functioning, history, needs, goals, and preferences.
- Works as part of a multi-disciplinary team to support health behavior change in persons with behavioral health challenges.
- Interacts and collaborates professionally with clients, their families, support systems, and community partners to assist in achieving treatment goals.
- Obtains and maintains required credentialing, including licensure or supervised status and Alcohol and Other Drugs Treatment Practitioner (AODTP) status.
- Maintains competency in instruments required by agency payers or commonly used by mental health/substance use professionals.
- Participates in staff consultations for case presentation, diagnostic, and treatment planning, as well as mandatory program meetings.
- Supports the monitoring and tracking of outcome measures for clients.
- Maintains clinical records in a timely manner as required by agency policy, funding sources, and accreditation authorities.
- Completes all required forms consistent with specified requirements.
- Maintains professional competency through appropriate readings, workshops, and trainings, and complies with requirements to maintain professional certification(s).
Requirements
- Master’s degree in social work, psychology, or a related human services field.
- Licensure in the State of Oklahoma in one of the mental health disciplines or under Board-approved supervision for licensure (e.g., LCSW, LPC, LMFT, LADC-MH, LBP).
- If licensed, must complete continuing education requirements for licensure maintenance.
- High degree of flexibility and efficiency.
- Proficiency in MS Word and intermediate general computer skills.
- Excellent interpersonal, verbal, and written communication skills in English.
- Ability to move about facilities freely.
- Casual professional dress adhering to dress code requirements.
- Access to reliable transportation and valid Oklahoma driver’s license; must provide verification during employment.
- Maintain and provide verification of current automobile liability insurance, including uninsured motorist coverage.
- Complete all NorthCare required training within 30 days of employment.
- TB test within 12 months prior to employment or obtained within 30 days of employment.
