About the role
Population Health and Practice Development Fellowship designed to train pharmacists in advanced clinical services, practice transformation, and community-based population health initiatives. The fellowship is structured to provide hands-on experience in implementing and managing clinical programs within an independent community pharmacy setting.
Responsibilities
- Develop, implement, and evaluate clinical service programs (e.g., pharmacogenomics, chronic care management, transitions of care, immunizations, point-of-care testing).
- Collaborate with healthcare providers, payers, and community organizations to integrate pharmacy services into broader population health strategies.
- Provide direct patient care through comprehensive medication management, chronic disease state management, and wellness consultations.
- Lead practice transformation initiatives to enhance workflow efficiency, patient engagement, and revenue diversification in the pharmacy.
- Participate in data collection, analysis, and reporting to demonstrate the impact of clinical services on patient outcomes and pharmacy performance.
- Engage in community outreach and education to promote pharmacy services and improve public health outcomes.
- Contribute to research, publication, and presentation efforts related to community pharmacy practice and population health.
- Mentor pharmacy students, interns, and staff in clinical service delivery and practice development.
Requirements
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.
- Active pharmacist licensure or eligibility for licensure in South Carolina.
- Completion of a PGY1 community pharmacy residency or equivalent experience in community-based clinical services (preferred but not required).
- Strong clinical knowledge in chronic disease management, immunizations, and preventive care.
- Experience with or willingness to learn pharmacogenomics, point-of-care testing, and collaborative practice agreements.
- Demonstrated leadership, communication, and project management skills.
- Commitment to advancing community pharmacy practice and improving population health.
