This position conducts all Pre-evaluation screenings for Lafayette County, assisting in other counties where needed, and attends all civil commitment initial hearings. The role acts as a court liaison between the Chancery courts and Communicare, completes intake assessments, and assists with care coordination for individuals whose needs can be met through outpatient services.

Responsibilities

  • Ensure communication with all agency staff regarding court/hearing dates
  • Successfully complete training required to be a Certified Pre-Evaluation Commitment Screener through the Mississippi Department of Mental Health
  • Utilize DSM-5 criteria to assess client symptomology
  • Develop rapport and a therapeutic alliance with clients and their caregivers, including being understanding and accepting of individual cultural/ethnic/religious differences and belief systems of clients served
  • Ensure adherence to the National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care
  • Provide motivational leadership, proficient in teambuilding, conflict resolution and strength-based management
  • Conduct outreach and engagement activities to reach individuals who may benefit from the program as well as related service provider/community resources
  • Provide clinical therapeutic targeted case management and additional clinical services where necessary
  • Become familiar with the intensive outpatient treatment modalities
  • Conduct data collection and assist with reporting as required by the Department of Mental Health
  • Actively collaborate with other agencies and supports regarding assigned individuals and their families
  • Act as a member of a multi-disciplinary team, monitoring participation and progress in treatment programs to assure the planned provision of service according to the individual's treatment plan
  • Provide referral services for those enrolled in the project when beneficial
  • Accompany individuals to appointments when necessary, which may include extensive travel throughout Region 2
  • Attend Chancery court hearings to discuss clients' treatment progress with the judge
  • Participate as a member of the AMAP Team and other advisory councils as appropriate
  • Participate in all required training, consultation, and evaluation procedures required by the project and the Department of Mental Health
  • Complete all mandatory documentation to monitor individuals' progress towards achievement of individualized treatment plan goals, grant related evaluations and associated documents
  • Complete and submit any requested reports to the Director of Intensive Services in a timely manner
  • Demonstrate appropriate interactions with coworkers that promote a pleasant work environment
  • Maintain client confidentiality at all times
  • Complete additional job duties assigned by the supervisor

Schedule

Flexible hours as needed, primary hours between 8-5 Monday through Friday with one hour for lunch.

Qualifications

  • Master's Degree in Social Work, Counseling, Psychology or a related field with current license or certification to practice counseling within the state of Mississippi
  • Previous experience in a mental health setting is strongly preferred
  • Ability to think independently is strongly preferred
  • Visual and auditory acuity sufficient to evaluate client/staff needs is preferred
  • Fine motor skills for legible and accurate writing of reports, scheduling, daily correspondence and presentations is preferred
  • Efficient use of time management is required

Required certifications and clearances

  • Valid MS driver's license, personal auto, liability insurance and Communicare insurable driving record
  • Acceptable criminal background check
  • Satisfactory completion of CPR prior to reporting to the work site
  • Satisfactory completion of CPI within 30 days of employment
  • Requires completion of annual trainings in compliance with DMH standards
  • Extensive travel throughout the region is required
