About the role

A County Caseworker 2 at Fayette County MH/ID supports individuals by helping them identify their needs and connect with the right resources. The role focuses on person centered planning and monitoring the quality of supports.

Responsibilities

  • Service Planning: Identify support needs and develop Individualized Service Plans that reflect personal preferences
  • Resource Linking: Locate and coordinate services, community supports, and natural supports that match each person's goals
  • Person Centered Support: Guide planning processes that respect individuality, choice, and control
  • Service Monitoring: Ensure services are delivered as planned and monitor the health and safety of individuals
  • Quality Oversight: Provide accurate information and support through ongoing review of services and outcomes

Requirements

This position requires a minimum of six months of experience as a County Caseworker 1, successful completion of the County Social Casework Intern program, a bachelor's degree with a social welfare major, or a bachelor's degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency. Applicants must be within two months of graduating with a qualifying degree/credits.

Other Requirements: PA residency requirement is currently waived for this title. Must possess a valid driver’s license which is not under suspension.

Schedule

Full-time employment. Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch. Telework: You will not have the option to telework in this position.
