About the role
This position offers intensive case management services to consumers and families in home and community settings. The role involves providing support during flexible hours and participating in on-call services for crisis intervention and support.
Responsibilities
- Provide Case Management: Deliver intensive support to consumers and families in their home and community settings
- Support During Flexible Hours: Offer assistance during evenings and weekends when necessary
- Participate in On-Call Services: Serve in a rotation to provide crisis intervention and support
- Collaborate with Families and Agencies: Connect them with community services
- Promote Family Stability: Help families navigate concerns and strengthen their support systems
Requirements
- Minimum Experience and Training Requirements: Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
- Other Requirements: PA residency requirement is currently waived for this title.
Qualifications
- You must be able to perform essential job functions.
- You must pass a background investigation.
