Learn more about us at www.dbhs.co. This opportunity is for County Caseworkers to build rapport and a professional relationship with consumers of all ages in services and their families. Serve as an advocate for the consumer, monitors the delivery of services, links individuals to services, develops social supports, and encourages self-determination.

Responsibilities

• Service Monitoring: Responsible for monitoring, linking, and evaluating services; link to other services and make referrals as needed.
• Direct Contact: Provide regular direct individual contact and face-to-face case management services while meeting productivity standards.
• Plan Development: Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family.
• Information Management: Enter necessary information and maintain information in computerized database, state computer systems, and charts in accordance with state, federal and licensing regulations and Agency policy; obtain & update information within required time frames.
• Crisis Support: Offer crisis intervention services and coordinate assistance as needed.
• Collaboration: Work with Agency staff, providers, and community resources to support individuals.
• Transportation Support: Link individuals to transportation services or transport individuals to appointments as needed.
• Attend meetings, staffings, and trainings as needed.
• May provide coverage in Bedford County.
