Responsibilities
- Assess and treat patients in the home to assist them in the development and execution of activities of daily living.
- Evaluate patients' functional status as well as the home environment to identify hazards or barriers to achieving more independent living.
- Perform appropriate diagnostic musculoskeletal, neurological, and functional skills tests to develop a treatment plan that establishes goals for improved function.
- Communicate the plan of care to the referring physician and collaborate with other clinicians to ensure patient needs are met.
- Educate patients on the use of various types of adaptive equipment to improve function.
- Provide instruction on proper posture and body mechanics to promote prevention of injury and maximize therapeutic outcomes.
- Educate patients on how to maximize quality of life within the limits of their physical capabilities.
- Provide guidance and education to family members and caregivers to ensure patients continue to sustain improvement with their physical, mental, or cognitive disorders.
- Document all changes in patients' condition and update care plans and outcomes accordingly.
- Supervise the care provided by Certified Occupational Therapy Assistants to ensure proper care is delivered.
- Instruct, supervise, and evaluate home health aide care every two weeks when therapy is the only skilled service.
- Participate in discharge planning for the patient.
