Bluestone Physician Services delivers exceptional care to patients living with complex, chronic conditions and disabilities through a unique, robust model of care. Our multidisciplinary care teams collaborate with patients, families, and other healthcare providers to deliver preventative, proactive, and tailored care. Using an evidence-based approach, Bluestone care teams manage chronic conditions, address social determinants of health, support transitions between care settings, and provide behavioral health support. Patients under our model experience 21% fewer ER visits, 36% fewer hospitalizations, and 41% fewer hospital readmissions compared to similar patients.
As a Nurse Practitioner (NP) or Physician Assistant (PA), you will provide on-site, team-oriented primary care to patients residing in Assisted Living and Memory Care communities in and around Owatonna, MN. You will manage a panel of approximately 200 patients (12-16 patients per day, with an average of 14), design your schedule, and develop care plans tailored to patients' needs. Clinical support is provided by Clinical Assistants, LPNs, RNs, and Social Workers, with access to diagnostics such as labs and x-rays. Collaboration with community partners, including home care and hospice, is also part of the role.
Full-time providers work Monday through Thursday, with Fridays available as a clinical or administrative day.
Responsibilities

- Collaborate with Clinical Assistants, LPNs, RNs, and Social Workers for clinical support.
- Utilize diagnostics such as labs and x-rays as needed.
- Work with community partners, including home care, hospice, and other resources.
Requirements
- Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
- Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
- Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
- Ability to obtain and/or maintain DEA certification for the state of work.
- Proven ability to communicate effectively with strong verbal skills.
- Excellent interpersonal and customer service skills.
- Valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required.
Preferred Qualifications
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant.

- Compensation: Base + Quality/Productivity ranging from $125,000 to $170,000.

$125,000—$170,000 USD
Schedule
