About the role
Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities. Full-time providers care for patients Monday–Thursday with the option for Fridays to be a clinical or administrative day. Manage a panel of approximately 200 patients (12–16 patients per day, averaging 14). You will have the ability to design and manage your own schedule, develop and implement individualized care plans, and collaborate with a multidisciplinary support team including Clinical Assistants, LPNs, RNs, and Social Workers. Diagnostics such as labs and x-rays are available as needed, and you will coordinate with community partners in home care, hospice, and other resources.
Responsibilities

- Develop and implement care plans tailored to patients' needs and preferences
- Collaborate with Clinical Assistants, LPNs, RNs, and Social Workers for care coordination
- Utilize diagnostics such as labs and x-rays as needed
- Partner with community resources including home care, hospice, and other services
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant education program
- Board Certified or Board Prepared through AANP, ANCC, or NCCPA
- Licensed or able to obtain licensure through the Board of Nursing or Board of Medical Practice
- Ability to obtain and/or maintain DEA certification for the state of work
- Proven effective communication with strong verbal skills
- Excellent interpersonal and customer service skills
- Valid, unrestricted driver's license and minimum state-required liability auto insurance; local travel required
Preferred qualifications
- 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant

Pay
$125,000–$170,000 USD per year (base + quality/productivity).
Schedule
Full-time, Monday through Thursday with the option for Friday to be a clinical or administrative day. Schedule design is flexible.
