Responsibilities

• Create the initial comprehensive and interdisciplinary patient-centered home health plan of care; update the plan in collaboration with the physician
• Effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of rehospitalization
• Supports agency established financial goals
• Demonstrate expert knowledge unique to the practice of community health nursing

Qualifications

• Excellent clinical assessment skills
• Strong ability to solve problems independently and interact with an integrated team
• Current NYS RN license and registration
• Bachelor’s degree in nursing preferred
• Minimum of one year acute medical-surgical nursing experience
• CHHA experience preferred
• Driver license and car required

Pay

$112,000.00 - $134,100.00 per year
