About the role

This position provides case management to Elderplan and HomeFirst members, focusing on medical, social, psychological, physical, and spiritual needs. The role involves developing, implementing, and monitoring care plans in collaboration with primary care physicians, caregivers, and other team members.

Responsibilities

• Develop, implement, and monitor care plans for members
• Collaborate with primary care physicians, caregivers, and other team members to address member needs
• Assess and address medical, social, psychological, physical, and spiritual needs
• Work closely with the Supervisor of Care Management to ensure effective care delivery

Requirements

• Associate’s Degree in Nursing for experienced candidates
• Bachelor’s Degree in Nursing for new graduates
• NYS RN license required
• CCM certification preferred
• One year of RN experience in managed care, home care, acute care, or nursing home preferred
