Coordinator Ambulatory Care Transitions - TGH Care Transitions

Essential Functions

• Plan and maintain the schedule and assignments for the team.
• Complete weekly timecards and payroll.
• Prepare and facilitate daily interdisciplinary huddles/reviews to improve and maintain quality patient outcomes within specified populations and programs.
• Plan and oversee the orientation of new team members.
• Contribute to the performance evaluation process.
• Maintain monitoring of business operations and productivity.
• Responsible for process improvement, care coordination, Remote Patient Monitoring (RPM) and Transitions of Care.
• Monitor program metrics and provide clinical supervision and oversight for the Care Transitions team.
• Support and guide Care Transitions team members and their patients in the health care decision-making process including but not limited to admissions, diagnostics, surgical/medical interventions, discharge, and post-acute support services.

Qualifications

• Bachelor's Degree in Nursing
• Active Florida Nursing License
• 3 years related work experience required; BSN may be completed within 2 years of hire from an accredited institution
