Coordinador(a) de Cuidado de Pacientes / Spanish Patient Care Coordinator

Full-time, in-person Patient Care Coordinator supporting our West Hempstead office and the local Hispanic community. Spanish fluency and basic/intermediate English required.

About the role

Axzons Homecare is seeking a highly motivated, energetic, and growth-oriented Patient Care Coordinator to join our team in West Hempstead. This role is key to increasing our patient census, strengthening relationships within the Hispanic community, and ensuring an excellent experience for patients and caregivers.

Responsibilities

• Serve as the liaison between patients, families, caregivers, and the agency.
• Help grow patient census through referrals, community visits, and relationships with Hispanic community leaders, families, and organizations.
• Communicate actively, clearly, and warmly with patients, caregivers, and agency staff.
• Make calls, follow up, and coordinate services.
• Assist with the intake process for new patients.
• Maintain accurate, organized records.
• Use computers, internal systems, and agency software efficiently.
• Identify growth opportunities to expand Axzons' presence in the community.
• Support office operations as needed.

Requirements

• Fluent in spoken and written Spanish (required).
• Basic/intermediate English communication, spoken and written (required).
• Excellent communication and people skills.
• Sociable, conversational, and relationship-oriented.
• Strong focus on goals, organization, and patient-census growth.
• Able to work independently and as part of a team.
• Comfortable with computers: email, document handling, and internal systems.
• Prior homecare, patient coordination, or customer service experience preferred but not required.
