About the role

Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care.

Responsibilities

  • Conduct one comprehensive initial assessment visit for each newly enrolled family, with capacity for up to approximately 35 assessments over the contract year.
  • Document TANF eligibility factors, including income, citizenship/immigration status, and household composition.
  • Assess family strengths and needs across multiple domains, including health, housing, food security, employment, and social support.
  • Administer the Edinburgh Postnatal Depression Scale (EPDS) or Patient Health Questionnaire-9 (PHQ-9) for depression screening.
  • Administer Intimate Partner Violence screening using the HITS (Hurt, Insult, Threaten, Scream) tool.
  • Administer the Ages and Stages Questionnaire (ASQ and ASQ:SE-2) for child developmental and social-emotional screening.
  • Administer Nursing Child Assessment Satellite Training (NCAST) tools or equivalent for parent-child interaction observation.
  • Initiate immediate safety planning when there are any safety concerns including but not limited to mental health or intimate partner violence.
  • Review assessment results with the family in a strengths-based, non-stigmatizing manner.
  • Provide assessment findings to inform the individualized family service plan developed collaboratively with the family and the Family Home Visitor.
  • Complete and submit assessment documentation in a timely manner to support program case records and TANF eligibility files.
  • Clock in and out with the Family Home Visitor and Director of Health Equity to complete assessments within an appropriate window of family enrollment.

Qualifications

  • Education & Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Minnesota required. Public health nursing experience or certification preferred.
  • Experience: Minimum 2+ years of experience in public health nursing, maternal/child health, home visiting, or a related clinical setting. Experience administering standardized screening tools (e.g., EPDS/PHQ-9, HITS, ASQ) strongly preferred. Experience working with low-income families and communities of color strongly preferred.
  • Knowledge, Skills And Abilities: Ability to build rapport quickly and deliver assessments in a culturally responsive, trauma-informed manner. Sound clinical judgment, including the ability to recognize and respond appropriately to safety concerns. Strong documentation skills and ability to meet agreed-upon reporting timelines. Ability to work independently while maintaining consistent communication with program staff.

Additional Requirements

  • Valid driver's license and access to reliable transportation required to travel to participant homes.
  • Must pass a criminal background check.
  • Must carry professional liability insurance or provide proof of coverage as required by contract.
