Contracted Case Manager

About the role

The case manager is responsible for administering and coordinating recovery support services for mental health, co-occurring and substance abuse clients. This position is part of a team of professionals who work together to provide a comprehensive array of therapeutic services to clients and their families.

Responsibilities

• Administer and coordinate recovery support services for mental health, co-occurring and substance abuse clients.
• Provide case management services to clients and their families.
• Collaborate with other professionals to ensure comprehensive therapeutic services.
• Coach individuals to reach their goals.
• Work with individuals on probation/parole, or those involved in specialty courts, if relevant.

Requirements

• Excellent oral, written, organizational, and record keeping skills.
• Proven, up-to-date knowledge of local community resources required.
• Ability to move from one task to another effectively and with ease.
• Ability to manage change with a positive attitude.

Physical Requirements

• Ability to move independently or with reasonable accommodation within the facility and community.

Qualifications

• Bachelor's degree in a human services field from a nationally-accredited university or college required.
• 1-2 years of experience providing case management services.
• Proven experience working with persons with diverse backgrounds.
• Experience successfully coaching individuals to reach their goals.
• Experience working with individuals on probation/parole, or with those involved in specialty courts, preferred.

• Excellent communication and interpersonal skills.
• Strong organizational and time management skills.
• Knowledge of community resources and services.
• Ability to work collaboratively with a multidisciplinary team.
