Responsibilities
- Provide direct care to individuals living with dementia, focusing on activities that enhance their quality of life.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans.
- Facilitate group activities and outings that promote social engagement and cognitive stimulation.
- Maintain accurate and detailed records of patient interactions and progress.
- Ensure compliance with all regulatory standards and policies related to memory care.
- Travel to clients' homes in a timely manner to provide consistent support.
Requirements
- Strong interpersonal and communication skills.
- Proven customer service experience.
- Background in Therapeutic Recreation, Allied Health, Social Work, or Creative Arts Therapy.
- Empathy and compassion for individuals with memory impairment.
- Strong documentation and organizational skills.
- Ability to collaborate effectively in a team setting.
- At least two years of experience working with the geriatric population.
- At least two years of experience working with individuals living with dementia.
- Ability to travel to clients' homes in a timely manner.
- Bilingual skills are a plus.
Qualifications
- Bachelor's degree in a relevant field such as Therapeutic Recreation, Allied Health, Social Work, or Creative Arts Therapy.
- Certification in a relevant field, if applicable.
- Valid driver’s license and reliable transportation.
- Current CPR certification.
