About the role

The Continuity of Care Liaison ensures uninterrupted, person-centered transitions between inpatient and community services by coordinating admissions, discharges, and post-discharge supports across State Mental Health Facilities (SMHFs), Contracted Psychiatric Beds (CPBs), Residential Treatment Centers (RTCs), and community providers. This role leads communication, scheduling, and linkage to services, to ensure individuals have timely follow-up care, medications, and support that promote recovery and community stability.

Responsibilities

  • Coordinate admissions, discharges, and post-discharge supports across various facilities and providers
  • Ensure timely follow-up care, medications, and support for individuals transitioning between settings
  • Lead communication and scheduling to facilitate seamless transitions
  • Link individuals to appropriate community services and supports

Requirements

  • Minimum Education: Bachelor’s Degree
  • Defined Education: Health or Human Services
  • Years’ Experience: Two (2) years
  • License/Certifications: QMHP or eligible to obtain QMHP credentialing

Qualifications

  • Strong communication and interpersonal skills
  • Ability to manage multiple tasks and prioritize effectively
  • Knowledge of mental health and healthcare systems
  • Experience with coordination and linkage to services
