Comprehensive Community Services- Human Services Professional II
About the Role
Under the direction of the CRS program manager and CCS supervisor, you will provide comprehensive, person-centered, team-based community treatment to youth and adults living with mental illness, substance abuse, or co-occurring disorders through the Comprehensive Community Services Program. This includes service facilitation, symptom management, psychotherapy, psychosocial rehabilitation, vocational supports, and skill development aligned with individualized Recovery plans.
You will deliver therapeutic interventions across the lifespan based on clinical assessments, maintain client case records, and collaborate with treatment teams, families, and collateral agencies in accordance with DHS 36, Social Security, and Medicaid/Medicare requirements.
Responsibilities
- 55% Psychosocial Rehabilitation Services:
- Manage a caseload and arrange/provide all needed services for youth and adults.
- Develop and implement culturally competent, recovery-oriented plans with consumer participation.
- Provide interventions such as assessment, service planning, medication management, physical health monitoring, skill development, employment training, psychoeducation, recovery support, and psychotherapy.
- Deliver services individually and in groups, including family support and education.
- Advocate for transitions to the least restrictive living situations.
- Attend and testify at court proceedings as needed.
- Assess consumer mental status, progress, and symptom patterns; communicate findings to the team.
- Develop community resources to support independent living.
- 25% Documentation and Compliance:
- Maintain consumer records, benefits, and reports per DHS 36, Social Security, and Medicaid/Medicare requirements.
- Assist with Medicaid/Medicare, Social Security reviews, FoodShare, and other community programs.
- Provide financial advocacy and budgeting support to increase consumer independence.
- Write clinically relevant recovery plans (six-month and annual), including Functional Screens and discharge plans.
- Complete initial/comprehensive assessments with yearly updates and other state reporting requirements.
- Document consumer mental status, progress, and challenges in detail for the treatment team.
- 15% Collaboration:
- Participate in recovery planning, supervision, and team meetings for assigned consumers.
- Provide clinical direction for consumers on your recovery team.
- Attend in-services, trainings, department meetings, and committee meetings as required.
- Maintain prompt communication with families, team members, crisis intervention, and collateral agencies.
- 5% Other Duties: Perform additional tasks as assigned.
Requirements
- Master’s degree in Social Work or a related field from an accredited institution.
- Preference for a state license in the degree field; consideration given to those working toward licensure.
- AODA knowledge and experience preferred.
- Valid, unrestricted driver’s license and auto insurance (per Rock County policies).
- Complete 20–40 hours of CCS and DHS 36 training within three months of hire.
Skills
- Assessment and treatment of mental illness/substance abuse disorders across the lifespan, including co-occurring disorders.
- Sound clinical planning based on assessments.
- Knowledge of social work principles, best practices, and psychosocial rehabilitation interventions.
- Familiarity with DHS 36 regulations, psychotropic medications, and community resources.
- Person-centered, recovery-oriented service delivery.
- Cultural competency and ability to work with diverse populations.
- Strong organizational, record-keeping, and reporting skills.
- Effective collaboration with agency staff, families, and community partners.
Working Conditions
- Physical demands: Standing, walking, sitting, lifting/carrying up to 25 lbs frequently.
- Work environments: Office, clients’ homes, and community locations; exposure to weather, unpredictable/aggressive individuals, and challenging situations.
- Transport consumers in your personal vehicle.
- Flexible schedule, including evenings and time adjustments for emergencies or care coordination.
- Potential exposure to physically/verbally abusive persons.
- Computer proficiency required (documentation systems, Microsoft Word).
