Relocation Package & Benefits

  • Starting Salary: $135,000 - $160,000

Job Specifics and Requirements

  • Location: Lake Charles, LA
  • Schedule: Full-Time
  • Education: Pharm.D.
  • Experience: 1+ years preferred, but new graduates are highly encouraged to apply!
  • Licensure: Must have (or be eligible to obtain) an open, active, and in good standing Louisiana State Pharmacist License.
  • Pre-Employment Screening: Ability to successfully complete a background check, health screen, and drug screen.

Job Responsibilities

  • Maintain productivity, quality standards, and legal compliance within a long-term care pharmacy setting.
  • Review and verify medication orders for long-term care facility residents to ensure accuracy and appropriateness.
  • Conduct comprehensive drug regimen reviews and identify potential medication-related issues.
  • Collaborate with physicians, nurses, and healthcare teams to optimize patient medication therapy.
  • Monitor for drug interactions, contraindications, and adverse effects.
  • Ensure compliance with federal and state regulations governing long-term care pharmacy operations.
  • Provide consultant pharmacist services and recommendations to facility staff.
  • Participate in interdisciplinary care planning meetings and provide clinical expertise.
  • Maintain accurate documentation and records in accordance with regulatory requirements.
  • Educate healthcare staff on proper medication administration and storage procedures.
  • Respond to medication-related inquiries and provide drug information support.
  • Oversee medication storage, handling, and disposal protocols within the facility.
  • Conduct quality assurance activities and participate in continuous improvement initiatives.
