Role Overview and Purpose

The Complex Care Resource Specialist supports Neighborhood Healthcare's mission and vision by coordinating patient referrals related to chronic care and social determinants of health (SDoH).

Responsibilities

  • Completes thorough referral processes according to standard procedures and protocol.
  • Tracks all referrals in designated logs and via eCW.
  • Documents all actions taken in patient medical records in accordance with current regulations and guidelines.
  • Discusses referral process with patients, including costs under provider direction.
  • Verifies insurance eligibility and submits treatment authorizations.
  • Administers SDoH program according to standard procedures and protocols.
  • Maintains accurate lists of specialists/providers by funding source and community resources.
  • Acts as liaison between patients, clinic/providers, specialty care providers, health plans/funding sources, and community resources.
  • Impacts patient experiences by demonstrating courteous and helpful behavior.
  • Shares accountability for overall patient health outcomes.
  • Attends meetings on and off-site as necessary.

Additional Qualifications (Knowledge, Skills, and Abilities)

  • Excellent verbal and written communication skills.
  • Knowledgeable about and experience with medical terminology.
  • Familiarity with computer skills, including spreadsheets and databases.
  • Ability to manage multiple tasks simultaneously.
  • Teamwork and independent work abilities.
  • Professional handling of confidential information.

Physical Requirements

  • Ability to lift/carry 25 lbs.
  • Standing for extended periods.

Compliance (Safety & HIPAA)

  • Follows safety procedures as outlined in IIPP and reports injuries and unsafe conditions.
  • Maintains current knowledge of policies and procedures.
  • Uses appropriate body mechanics for an injury-free environment.
  • Familiarizes with fire extinguisher and emergency exits locations.
  • Maintains privacy of all patient, employee, and volunteer information.
  • Reports unethical, fraudulent, or unlawful behavior.

Pay Range

$22.88 to $32.04 per hour, depending on experience, education, and additional qualifications.
