About the role
The Community Transitions Navigator (CTN) provides healthcare navigation services to enrolled clients/patients.
Responsibilities
- Coordinates medical home placements and first appointments.
- Collaborates with a multi-disciplinary team to address the social determinants of health.
- Arranges supportive services and referrals to community partners for improved health outcomes and to prevent avoidable hospital admissions/emergency department visits.
- May provide limited health coaching and advocacy to improve assigned clients'/patients' appropriate healthcare utilization.
- Assists clients/patients with applications for healthcare payor options, prescription assistance and/or other benefit programs.
- Attends appointments and makes home visits as required to advocate for the client/patient and connect them to needed resources.
Job Qualifications
- Associate degree in Human Services, Sociology, Psychology.
- Required: Bachelor’s degree in related fields.
- PREFERRED: Associate degree in related fields with 5 years of experience in healthcare, care coordination or case management.
