Community Support Specialist

Job Duties And Responsibilities

• Supporting individuals with community participation activities such as mobility and pedestrian safety, community resource use and access, and leisure, recreation, and socialization planning.
• Collaborating to arrange health care, social services, and appointments, and providing guidance and support to complete self-care tasks.
• Cueing, guiding, supervising, and training to complete routine household care and maintenance, and advocating for tenants.
• Implementing crisis prevention skills, problem-solving, and support strategies for self-sufficiency.
• Aiding in defining work goals, discovering personal strengths, and interests, and assisting with job search resources and supports.
• Assisting with job analysis, coaching, and training to strengthen work skills, behaviors, and coworker relationships, and arranging for adaptive accommodations and assistive technology.

Requirements

• Must be over the age of 18 years.
• Must successfully clear a DHS background check.
• Must have a current driver’s license, auto insurance, reliable transportation, and an acceptable driving record.
• Must be patient, compassionate, flexible, dependable, and passionate about helping others.
• Must be able to communicate effectively, demonstrate strong interpersonal skills, and form trusting relationships with individuals served.
