Community Support Specialist
Summary
Betty Jean Kerr Peoples Health Centers is seeking a Community Support Specialist (CSS) to join our team. The CSS will provide support to individuals at risk of hospitalization or recently discharged from inpatient treatment, helping them adjust to life in the community. Responsibilities include evaluating clients, developing person-centered plans, monitoring progress, and assisting clients in achieving their goals.
Requirements
- Bachelor's degree and two years of qualifying experience
- Any four year combination of higher education and qualifying experience
- Four years of qualifying experience, including:
- Delivery of service to individuals with mental illness, substance abuse, or developmental disabilities
- Providing one-on-one or group services with a focus on coping and managing psychiatric, developmental, or substance abuse issues
- Encouraging the use of natural resources and supporting efforts to find and maintain employment
- Able to assist individuals in achieving goals outlined in their individualized treatment or person-centered plans
Qualifications
The ideal candidate will have a strong commitment to supporting individuals in their recovery journey and will possess excellent interpersonal skills. They should be able to work independently and collaboratively with a multidisciplinary team.
