COMMUNITY SUPPORT SPECIALIST

Position Summary

Supportive Concepts for Families is seeking compassionate, dependable, and motivated individuals to join our team as Direct Support Professionals (DSPs). In this rewarding role, you will provide direct support to individuals with intellectual and developmental disabilities (IDD), helping them achieve greater independence, build life skills, and actively participate in their communities.

Essential Responsibilities

• Provide transportation to medical appointments, benefit and financial appointments, housing-related appointments, community activities, educational programs, and religious services.
• Support individuals in home and community settings with daily routines and activities.
• Aid in activities of daily living (ADLs), including personal care, hygiene, meal preparation, and household tasks.
• Administer medications in accordance with agency policies and training requirements.
• Implement Individual Support Plans (ISPs) and support individual goals and outcomes.
• Ensure the health, safety, and well-being of individuals receiving services.
• Document services accurately and complete all required paperwork, including: progress notes, daily logs, medication records, schedules, assessment forms, medical appointment forms, vehicle logs, expense reports, time records.
• Collaborate with Program Coordinators and interdisciplinary team members to ensure individual needs are met.
• Provide support during emergencies and participate in on-call responsibilities as assigned.
• Assist with special projects and program-related initiatives as needed.
• Promote a safe, respectful, and supportive environment that encourages personal growth, independence, and community participation.

Qualifications

• Must be at least 18 years of age.
• Valid driver's license required. Access to a reliable, insured vehicle suitable for travel between service locations.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Ability to provide hands-on assistance and physical support when needed.

Preferred Qualifications

• High School Diploma or GED.
• Six months to one year of experience in a human services, caregiving, healthcare, education, or related field.
• Experience working with individuals with intellectual and developmental disabilities.
• Knowledge of person-centered supports and community-based services.
