South Jersey Behavioral Health Resources, Inc. is seeking a compassionate and experienced Community Support Services Licensed Practical Nurse to provide nursing services, health education, and mental health rehabilitation support to individuals enrolled in our Community Support Services program. This is an opportunity to use your clinical expertise to help individuals maintain their physical and psychiatric stability, strengthen their independent living skills, and achieve greater success in the community. You will work as an important member of an interdisciplinary team committed to delivering respectful, person-centered care.

Responsibilities

  • Provide nursing services and health education to individuals enrolled in the Community Support Services program.
  • Deliver a minimum of five hours per day of direct, face-to-face nursing and/or mental health rehabilitation services in accordance with each individual’s rehabilitation plan.
  • Document observations, services, consumer interactions, and required follow-up activities accurately and in accordance with Community Support Services documentation standards.
  • Administer and monitor medications.
  • Monitor individuals for medication side effects and report problematic reactions to the appropriate supervisor and treating physician.
  • Educate individuals and their families about medications and other health-related matters.
  • Serve as the primary nurse for assigned individuals and help support their continued medical and psychiatric stability.
  • Coordinate care with medical providers and accompany individuals to appointments when appropriate.
  • Advocate for individuals and teach them how to advocate effectively for their own healthcare needs.
  • Contribute current information regarding individuals’ mental health, physical health, behavior, and progress toward rehabilitation objectives as a member of the interdisciplinary team.
  • Provide on-site support and supervision.
  • Encourage the development of skills needed for successful and sustainable community living.
  • Review laboratory results and medical records and report abnormal findings to the appropriate supervisor and ordering physician.
  • Contact third-party payors to obtain prior authorization for medical procedures.
  • Complete monthly clinical record audits.
  • Participate in quality improvement activities and management meetings when necessary and appropriate.
  • Complete all mandatory and assigned training successfully.

Requirements

  • Current and unrestricted New Jersey Licensed Practical Nurse license.
  • Graduation from an accredited school of practical nursing.
  • At least two years of licensed nursing experience.
  • Valid driver’s license.
  • Driving record that meets the requirements of the organization’s insurance carrier.
  • Ability to read, write, speak, understand, and communicate in English sufficiently to perform the responsibilities of the position.
  • Ability to use the electronic, computer-based, and manual information and recordkeeping systems required by the organization.
  • Ability to complete all mandatory training associated with the position.

Preferred Qualifications

  • Previous mental health or behavioral health experience.
  • Experience working with individuals who have a significant history of hospitalization.
  • Experience providing community-based nursing or rehabilitation services.

Training Requirements

Upon hire, the selected candidate must successfully complete:

  • South Jersey Behavioral Health Resources, Inc. orientation.
  • Competency-based adult mental health rehabilitation program training.
  • CPR certification.
  • First Aid certification.
  • Additional training assigned according to the requirements of the position.

Pay

$27.80 per hour.

Schedule

  • Full-Time.
  • Monday through Friday, 10:00 a.m. to 6:00 p.m.
  • Saturday and Sunday coverage on a six-week rotational basis.
  • Morning and afternoon schedule flexibility may be available.
  • Schedule flexibility may be required to meet program and consumer needs.

Physical and Functional Requirements

  • Ability to travel as required to support individuals and attend appointments.
  • Ability to perform documentation and use required electronic and manual recordkeeping systems.
