Community Support Navigator

Role Overview: The Community Support Navigator provides recovery support and community services to clients with mental health, substance use, dual diagnosis, and co-occurring disorders. This full-time, hybrid role involves care management, system navigation, and building rapport with clients in community settings, clinics, and other VOAMASS site locations. Training is provided upon hire, with no certification required initially. Key Responsibilities Engage clients post-admission to facilitate access to treatment and support services.Conduct assessments, develop service plans, and assist clients in achieving their goals.Refer and connect clients to resources such as healthcare, housing, employment, and social supports.Follow up to ensure appointment attendance and treatment engagement.Coordinate with referral providers and team members to deliver integrated care.Maintain accurate documentation according to policies and third-party requirements. Qualifications & Skills Bachelor’s Degree preferred in psychology, social work, healthcare administration, or related fields.At least two years’ experience in social services, case management, and care coordination.Excellent communication, organizational, and computer skills.Valid driver’s license and reliable transportation.Knowledge of local resources in the MA area is desirable.Training for co-occurring support provided upon hire. Additional Details: The role involves travel to various locations, community outreach, and case-by-case telehealth use.
