About the role
This position involves obtaining and maintaining proper certification to perform CSS, managing the certification process independently, and providing comprehensive case management services.
Responsibilities
- Meet productivity requirements by planning ahead, developing a schedule, and seeking support when needed.
- Provide case management services for clients who qualify for Community Support Services.
- Ongoing assessment of clients' needs and development of appropriate treatment plans.
- Cookordination of case management and mental health center services provided to clients, including services from other agencies.
- Provide supportive counseling to clients and their families as needed.
- Assist clients in making applications for benefits and assistance, securing housing, and managing medications.
- Afford access to mental health and other community services as needed.
- Perform home visits as determined by the level of client need.
- Provide advocacy services for CSS clients.
- Offer family counseling and education concerning mental illness to family members of CSS clients.
- Maintain a case load as assigned, including proper documentation and paperwork.
- Attend meetings and training related to CSS as designated by the Director of Community Support Services.
- Complete and turn in reports and progress notes as requested.
- Participate in grant-specific activities, including continuation of funding requests, meeting program objectives within funding restraints, coordinating and reporting on Outpatient Commitment Orders, and actively engaging in networking opportunities with area hospitals and AMAP team.
- Perform other duties as assigned.
Requirements
- Bachelor's degree in social work, counseling, psychology, or a related field.
- Ability to relate effectively to severely mentally ill individuals.
- Transportation, valid driver's license, automobile insurance, and a safe driving record.
- Maintenance of a Communicare insurable driving record.
