About the role
The Community Services Support Professional provides direct care through implementation of Habilitation Plans, promoting independence, community integration, and overall well-being.
Responsibilities
- Provide supervision and all direct care required to maintain safety and comfort of the person receiving services
- Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration
- Develop and plan of inclusive community activities
- Establish and maintain positive relationships with area businesses
- Aid people receiving services with completing daily or routine domestic or personal care tasks
- Foster and promote skills leading to independent living
- Assist with the development of Habilitation Plans and following these plans for training and assistance
- Train and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required
- Supervise people supported during program hours and during community outings as to ensure their health and safety
- Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices
Requirements
- A High School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred
- Possess a valid driver’s license acceptable to the agency’s insurance carrier
