About the role

The Community Residence Counselor (CRC) provides direct patient care and participates in the therapeutic milieu within a residential program serving adults (18+) with moderate to severe or treatment-resistant OCD. This position operates under the direction of the Program Manager and requires remaining awake and alert at all times while on duty.

Responsibilities

  • Provides direct patient care in accordance with current treatment plans, under supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
  • Supervises and facilitates community activities tailored to patient needs.
  • Assists patients in maintaining and attending appointments.
  • Documents care, observations, and shift notes clearly, concisely, and in a timely manner.
  • Establishes and maintains therapeutic relationships with patients while upholding professional boundaries.
  • Participates in patient rounds, status checks, and team meetings as outlined by program guidelines.
  • Completes assigned patient care tasks per program guidelines or daily assignment sheets.
  • Manages the therapeutic milieu effectively, providing direction and supervision to patients as needed.
  • Follows established hospital and program policies, procedures, and protocols.
  • Communicates observations about the patient milieu, therapeutic environment, medication side effects, and physical or emotional changes to appropriate staff promptly.
  • Encourages and supports patients in activities of daily living.
  • Assesses sleep patterns and assists patients in developing healthier sleep cycles, notifying the clinical team as necessary.
  • Completes mandatory annual in-service requirements, including CPR and de-escalation training.
  • Attends and participates in rounds and case conferences when appropriate.
  • Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.
  • Demonstrates physical ability to participate in patient activities.

Requirements

  • High School Diploma or equivalent required; Bachelor’s Degree in a related field preferred.
  • Medication Administration Program (MAP) certification from the Massachusetts Department of Public Health required.
  • Class D Passenger Vehicle Driver’s License preferred.
  • One year of patient care or related experience preferred, ideally in a psychiatric setting.
  • Basic computer skills to use electronic systems (e.g., PRISM) for communication, clinical, and administrative purposes.
  • Familiarity with Epic preferred.
  • Ability to drive and transport patients may be required based on role and location.
