Community Relations Assistant

QUALIFICATIONS

• High School Diploma
• CNA or CHHA certification preferred
• At least one year of healthcare experience preferred
• Bilingual preferred

FUNCTIONS & RESPONSIBILITIES

• Works in conjunction with the admission team
• Establishes presence of the organization to service members in the field
• Responds to referrals for evaluation and/or admission to the organization's program
• Explains the program to patients, family members, and other referral sources
• Works with admission team for referrals, admissions, and care coordination of patient and family needs
• Identifies key referral sources and assists the admission counselors to develop a marketing plan for the program
• Attends and participates in in-services, staff meetings, and case conferences
• Completes all documentation in an accurate and legible manner
• Complies with company policies and procedures, laws, and regulations, and standards of practice
