About the role
The Community Paramedic delivers high-quality in-home clinical care as part of Total Care Connect’s Mobile Integrated Healthcare programs. This role focuses on proactive patient care for high-risk populations, supporting hospital readmission reduction, emergency department avoidance, and improved chronic disease management. Community Paramedics conduct scheduled patient visits, perform clinical assessments, deliver care within established protocols, and collaborate with virtual providers and interdisciplinary care teams to provide coordinated, patient-centered care.

Required Qualifications
- Current Ohio Paramedic certification in good standing
- Minimum 2–3 years of paramedic field experience
- Valid driver’s license with acceptable driving record
- Strong clinical assessment and patient education skills
- Experience using electronic documentation systems
Work Environment
- Field-based role providing care in patient homes, partner offices, or community clinical settings.
- Travel between patient homes, partner offices, and operational locations within the assigned region
- Exposure to varied home environments including pets, limited lighting, clutter, stairs, and environmental hazards.
