About the role
Community Paramedic responsible for delivering scheduled, patient-centered care in home and community settings.
Responsibilities
- Conduct in-home assessments, treatments, and follow-ups under physician protocols
- Care for patients with chronic conditions or post-acute needs
- Serve as a link between EMS, primary care, and specialists
- Document care in electronic records
- Connect patients to resources and follow-up care
Qualifications & Skills
- Pennsylvania Paramedic certification (CP-C preferred)
- Minimum 2 years field experience preferred
- Valid driver's license
- Strong clinical judgment
- Effective communication
- Independent work ability
- Commitment to patient-centered care outside traditional EMS
