Essential Functions
- Recruits a client base within the community by identifying, locating, interviewing and screening individuals who may be appropriate for the program.
- Provides general information to individuals and families on program objectives and services, eligibility requirements and benefits, confidentiality of information, etc.
- Distributes informational materials and literature.
- Schedules clients for appointments with health care providers. Reminds them of pending appointments and contacts them to inquire into reasons for missed appointments. Escorts clients to various appointments to ensure compliance and provide support.
- Aids clients in obtaining necessary transportation for appointments.
- Aids with client retention by following up on all contacts by telephone or by a home visit follow-up. Locates clients who have moved or lost contact with the program.
- Serves as a liaison between the client and community resources including department staff, City, State, and Federal social services agencies.
- Aids with scheduling and participating community special events such as health fairs, workshops, etc.
- Participates in staff meetings and conferences. Reports on community outreach activities including problems and concerns.
- Achieves monthly, quarterly, annual enrollment goals, determined by yearly program funding.
Education and Experience
- High school diploma or GED.
- Minimum two years' experience providing outreach services.
- Experience should include writing reports and maintaining records.
- Effective oral and written communication skills.
- Ability to establish and maintain effective working relationships.
- Ability to handle sensitive and confidential matters with discretion and tact.
Total Rewards
The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the successful candidate's geographic location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/training and other qualifications.
