Community Outreach Associate
Full Time
Service areas (Atlantic, Cumberland and Salem counties)
$20.00 per hour
Responsibilities
- Work with individuals in the community to support community goals, recreational goals, and social environment activities
- Serve as an advocate for program participants to be fully integrated into community life
- Train, assist, and/or supervise individual(s) in areas such as daily living, personal care, financial management, community and social integration, medication administration, and job skills development
- Establish a therapeutic relationship with the individual receiving services to ensure effective guidance, support, and service delivery
- Work on behavioral assistance and family systems; work with family members of individuals in program
- Achieve objectives in accordance with the service plan
- Maintain current progress and service notes; complete and submit required documentation to direct supervisor in accordance with applicable policies and deadlines
- Provide verbal feedback and data relevant to individuals’ services, service plans, or administrative policy
- Meet with employers to develop potential work opportunities for individuals receiving services
- Attend training and other meetings as required
- Provide emergency coverage and/or care for individuals receiving services; report incidents of maltreatment, medical, and behavioral concerns in accordance with company policy
- Maintain confidentiality, respect human rights, and practice universal precautions in accordance with company standards
Qualifications
- High school diploma or equivalent required; Bachelor’s degree preferred
- Two years of experience in human services preferred
- All state-required training and certification completed in mandated timeframes
- Valid driver’s license in good standing with car registration and vehicle insurance
- Ability to maintain composure and exercise sound judgment and discretion
- Reliable, responsible, and caring nature

- Exceptional communication skills with an ability to establish trust and rapport quickly
- A good listener with an ability to empathize while still providing guidance
