Community Outreach Associate
Full Time
Service areas (Atlantic, Cumberland and Salem counties)
$20.00 per hour

Responsibilities

• Work with individuals in the community to support community goals, recreational goals, and social environment activities
• Serve as an advocate for program participants to be fully integrated into community life
• Train, assist, and/or supervise individual(s) in areas such as daily living, personal care, financial management, community and social integration, medication administration, and job skills development
• Establish a therapeutic relationship with the individual receiving services to ensure effective guidance, support, and service delivery
• Work on behavioral assistance and family systems; work with family members of individuals in program
• Achieve objectives in accordance with the service plan
• Maintain current progress and service notes; complete and submit required documentation to direct supervisor in accordance with applicable policies and deadlines
• Provide verbal feedback and data relevant to individuals’ services, service plans, or administrative policy
• Meet with employers to develop potential work opportunities for individuals receiving services
• Attend training and other meetings as required
• Provide emergency coverage and/or care for individuals receiving services; report incidents of maltreatment, medical, and behavioral concerns in accordance with company policy
• Maintain confidentiality, respect human rights, and practice universal precautions in accordance with company standards

Qualifications

• High school diploma or equivalent required; Bachelor’s degree preferred
• Two years of experience in human services preferred
• All state-required training and certification completed in mandated timeframes
• Valid driver’s license in good standing with car registration and vehicle insurance
• Ability to maintain composure and exercise sound judgment and discretion
• Reliable, responsible, and caring nature

• Exceptional communication skills with an ability to establish trust and rapport quickly
• A good listener with an ability to empathize while still providing guidance
