About the role
This is a hybrid position that supports occasional in-person outreach in the Harrisburg, PA area. 8 am to 5 pm Monday thru Friday EST.
Responsibilities
- Engage patients via telephone in a friendly, supportive, and professional manner
- Engage difficult-to-reach patients through in-person approaches to drive higher acceptance rates
- Conduct high-volume outreach to prospective patients, building rapport and creating meaningful engagement through consultative conversations
- Clearly communicate the value and benefits of ConcertoCare's services, addressing questions and overcoming barriers to enrollment
- Drive patient enrollment by identifying needs, handling objections, and motivating patients to take action
- Utilize creative outreach strategies, including phone, in-person, and community-based engagement, to connect with difficult-to-reach patients
- Manage a pipeline of prospective patients and prioritize activities to maximize enrollment opportunities
- Track outreach activities, enrollment progress, and patient interactions in Salesforce and electronic medical record systems
- Schedule initial patient appointments and coordinate required intake activities to ensure a smooth onboarding experience
- Respond to inbound inquiries from prospective patients and caregivers, educating them on available services and guiding them through enrollment
- Collaborate with clinical and operational teams to remove barriers and ensure successful patient engagement
- Maintain the highest standards of professionalism, patient confidentiality, and regulatory compliance
Requirements
- 1 year of experience in a healthcare, patient-oriented position
- 1 year of experience in providing customer service
- Sales experience and history of consistently achieving sales goals/metrics preferred
- Appointment scheduling experience
- CNA/MA background is a plus
- Call center experience is a plus
- Demonstrated passion for extraordinary patient service
Qualifications
- Driver requirements: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Pay
Base Salary/Wage Range: $21.00 to $24.00 plus competitive incentive plan. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
Schedule
8 am to 5 pm Monday thru Friday EST
