Position Summary

Mindify Wellness & Care is seeking an outgoing, professional, and relationship-driven Community Outreach Representative to expand our community presence throughout all of New Jersey, with a focus in Central NJ. This role serves as a key ambassador for Mindify, introducing our services to adult day care centers, physician offices, and community organizations, identifying referral opportunities, scheduling partnership meetings with leadership, and fostering long-term relationships through ongoing in-person engagement.

Community Outreach & Business Development

  • Travel throughout New Jersey visiting adult day care centers, physician offices, senior centers, and community organizations.
  • Introduce Mindify's behavioral health services and educate potential referral partners on the value of our programs.
  • Build meaningful relationships with administrators, Directors of Nursing (DONs), physicians, office managers, social workers, and case managers.
  • Identify qualified partnership opportunities and schedule meetings with Mindify leadership.
  • Represent Mindify at networking events, community gatherings, and industry functions.

Relationship Management

  • Conduct regular in-person check-ins with existing referral partners.
  • Maintain strong communication and cultivate long-term professional relationships.
  • Cookordination follow-up activities with leadership and clinical staff as needed to support partner success.

Community Education

  • Deliver presentations highlighting Mindify's services and impact.
  • Educate partners on referral processes, behavioral health resources, and available support services.

Documentation & Reporting

  • Accurately document outreach activities, meetings, referrals, and follow-up efforts within the CRM system.
  • Submit weekly outreach reports and provide market insights and feedback.

Qualifications

  • High school diploma or equivalent required.
  • Excellent communication, interpersonal, and relationship-building skills.
  • Valid driver's license and reliable transportation.
  • Bilingual (English/Spanish) preferred.
  • Experience in healthcare outreach, marketing, customer service, community engagement, or business development preferred.

Compensation

  • Salary: $55,000–$60,000 annually, based on experience.
  • Eligible for a semi-annual performance bonus of up to $5,000 every six months (up to $10,000 annually).
