About the role
We're committed to bringing passion and customer focus to the business. The Community Outreach Representative engages Medicare seniors to create organic patient growth in local communities, working closely with community partners and clinics to ensure seniors have access to value-based preventive primary care. This role reports to the local Outreach Community Manager and operates in a hybrid model (50% field/home office).
Responsibilities
- Deliver and coordinate educational presentations to existing and prospective patients.
- Create and maintain working relationships with referral sources, local brokers, and community partners (e.g., non-profits).
- Attend lead-generating events in the community and at clinics.
- Engage potential members through phone, text, and email.
- Manage and report on leads through CRM or other tracking tools.
- Collaborate with health plan partners, marketing, and clinic staff to promote events and services.
- Conduct prospective patient tours in partnership with operations.
- Convert prospective patients to enrolled/active patients with booked visits and assigned PCP.
- Develop and execute social media and content strategies with marketing.
Requirements
- Bachelor’s degree in business, sales, marketing, or equivalent work experience.
- At least two years of experience working in the healthcare sector.
- At least one year with Medicare; Value-Based Care experience is a plus.
- Experience working in a quota-based compensation model.
- Proficiency with PowerPoint and presentation skills.
- Experience with reporting and tracking through Excel.
- Excellent written and verbal communication skills.
Pay
Base Salary: $60,000–$65,000, plus incentive compensation (total earnings up to $120,000 based on performance).
