About the role
HRMC is looking for applicants to help bridge the gap between the community and health and social service system by providing one on one support to individuals and families.
Responsibilities
Enhance communication between community members, healthcare resources, organizations, and businesses through informal education, prescription assistance, and assistance navigating the healthcare system. Maintain and practice cultural competency and integrity within the community. Demonstrate the mission of HRMC by displaying the REACH core values: Respectful, Engaged, Available, Competent, and Helpful.
Requirements
- Certification or willingness to obtain Community Healthcare Worker credential from an accredited college
- Valid driver’s license and reliable personal transportation
- Familiarity with community resources, the 211 program, and local healthcare services
- Excellent written, verbal, and non‑verbal communication skills

Schedule
Shift: 8:00 am – 4:30 pm; Job Schedule: Full time; Weekly Hours: 40 hours
