Responsibilities
- Provide active engagement and support for all individuals assigned, including community integration, training, leisure, communication, behavior, personal care, bus loading/unloading, medication, safety, and risk management.
- Develop and implement Personal Centered Individual Service Plans (ISP) including social, community inclusion, communication, behavior, and skill development.
- Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and CARF as well as other funding sources and company policy/procedure.
- Maintain participant records, daily notes, quarterly reports, etc. per Medicaid, Licensure and CARF standards.
- Communicate with family members, guardians, authorized representatives, residential providers and Support Coordinators regarding participant’s illness, incidents, progress, etc. at the direction of the Program Manager (or designee).
- Complete incident report as required.
- Inform the Program Manager (or designee) of missing or inaccurate information immediately.
- Participate in department, participant, and staff committee meetings.
- Participate in trainings as required by funding sources and the corporation.
Qualifications
- A High School diploma (or GED) required; plus, one (1) year of experience working with adults with developmental disabilities or the related field.
- Preferred: High School diploma (or GED) plus a combination of college level courses in the human services field and experience working with adults with developmental disabilities or a bachelor’s degree in the human services field and experience working with individuals with developmental disabilities.
Additional Duties and Responsibilities
- Maintain participant records, daily notes, quarterly reports, etc. per Medicaid, Licensure and CARF standards.
- Adhere to health and safety regulations.
- Participate and/or provide input in the PCP process (plan, quarterlies, daily notes) and work in conjunction with the team in the implementation, monitoring and review of the individual’s written plan.
- Utilize appropriate behavior intervention techniques, proper lifting, feeding, and transferring techniques as written.
- Complete and pass the Orientation to Development Disabilities (ODD) test and competencies.
- Perform related duties and responsibilities as assigned/requested.
Physical Demands
- Stand; reach with hands and arms; talk or hear.
- Occasionally walk and sit.
- Lift or move products and supplies, up to 50 pounds.
