We are one of the state’s largest private, multi-service agencies dedicated to meeting the needs of children and adults with all types of disabilities.

Responsibilities

  • Person-Centered Planning and Documentation (5%): Assist clients in developing and maintaining individualized community inclusion plans using a person-centered approach. Complete and submit required documentation, including semi-annual progress reports, in accordance with program and regulatory requirements. Ensure all services and supports align with each client’s community inclusion plan, goals, interests, strengths, skills, and preferred activities.
  • Community Inclusion and Skill Development (80%): Provide community inclusion services and supports to individuals or small groups of up to three clients with similar interests or goals. Support clients in identifying, accessing, and initially engaging in activities that promote socialization, education, recreation, and personal growth. Assist clients in developing, practicing, and applying skills that increase independence, community participation, and meaningful relationships with non-paid community members.
  • Community Engagement and Collaboration (10%): Build and maintain positive working relationships with clients, community partners, and service providers to support client participation and inclusion in the community. Travel between work sites, attend off-site meetings, and engage with clients and community partners as needed to effectively fulfill job responsibilities.

Qualifications

  • HS Diploma/GED and 1 work year experience
  • Experience working with individuals with disabilities in a community setting is preferred
  • Familiarity with the community in which the participant resides, including knowledge of community organizations, informal clubs, community projects, and businesses
